Why is this used Past Perfect?

WHY is this used Past Perfect?
Got and Gotten,both are past perfect form.I got it.
But still I'm confused about this sentence.
I think 'I saw...' and 'Livia had got...' both should be used same tense.Because these happened same time....
I'm really confused.Help me.
You should have used had gotten, but a better choice might be had climbed.
Head first should be hyphenated: head-first.
